Skillnet Event Registration Forms

The forms attached below are required by Irish Medtech Skillnet and are audited by Skillnet Ireland
Please ensure that all trainees are notified that these forms are collected for Skillnet Ireland's statistical purposes only. The purpose of this form is to gather profile information about participants on programmes co-funded through Skillnet Ireland and member companies from the National Training Fund through the Department of Education & Skills (DES).

Only aggregated participant data will be reported to the DES.

Irish Medtech Skillnet will comply with all applicable data protection legislation in respect of the information and personal data provided by you in this form. You have the right of access to the data by means of a written request and you can request Irish Medtech Skillnet to correct any inaccuracies in the data.

Skillnet Ireland periodically conducts surveys to evaluate the quality and impact of the programmes delivered by its training networks. As a result, you may be contacted in future and asked to participate in such a survey.

The form are now digital online forms and the link to complete registration and feedback is:

Any hard copy registration and feedback forms completed must be returned after the Skillnet funded training / event to;

• Irish Medtech Skillnet Introduction Slide - This slide is introduced at the beginning of each event funded through the Irish Medtech Skillnet.
• Sign In Sheet - All delegates must sign in to event funded through the Irish Medtech Skillnet.
• Irish Medtech Skillnet Registration Form - All delegates must complete registration form on day of event funded through the Irish Medtech Skillnet.
• Trainee Evaluation Form L1&2 - All delegates must complete an evaluation form at the end of event funded through the Irish Medtech Skillnet.
• Trainee Long Term Evaluation Form - Should be filled out by each trainee 6 to 8 weeks following the completion of the event.
• Supervisor Feedback Form - Should be filled out by each trainees supervisor 6 to 8 weeks following completion of event.
